FIRE RELIEF GROCERY VOUCHER FORM

THIS IS ONLY FOR UNION MEMBERS WHO HAVE BEEN IMPACTED BY THE RECENT LA FIRES (PALISADES & EATON - PASADENA & ALTADENA) WHO EITHER LOST THEIR HOME, EXPERIENCED DAMAGE TO THEIR HOME OR WHOSE CURRENT EMPLOYMENT WAS IMPACTED.

The LA County Office of Food Systems , in partnership with Labor Community Services, has a limited amount of $500 grocery vouchers to support individuals and households impacted by the 2025 LA wildfires. The vouchers will be distributed on Saturday, May 31, 2025.

Through this program, eligible households will receive vouchers that can be redeemed for groceries at participating local stores, providing direct relief and ensuring families can access the food they need during this challenging time.

You have been identified as someone who may have been impacted by the wildfires through the loss of a home or job and might qualify for a $500 grocery voucher.

PLEASE NOTE: This invitation is ONLY for you. It cannot be passed along to anyone else. It is only being offered to people who are pre-qualified.

NEXT STEPS:

1. Fill out the form below to sign up for your appointment at UFCW Local 770, 630 Shatto Pl., Los Angeles, CA 90005. You will be asked to select a timeslot to pick up your grocery voucher between 9:00am and 1:00pm on Saturday, May 31, 2025. We encourage you to arrive 15 minutes prior to your selected time slot.

2. At the end of the form, you will be asked to upload ONE form of photo ID. The forms of ID that are accepted are:
